1. Introduction

This Privacy Policy explains the information we collect and process using the official Level Five Application and related Website (collectively, the “Service”). It also details your rights and choices concerning your personal information.

When we refer to “Innovation Health Benefits TM,” “We,” “Us,” or “Our” in this document, we are referring to Socially Seamless of info@innovationhealthbenefits.com, the “Data Controller” for this Service.

The Data Controller determines the purposes and means of processing your personal data and the content and related services or features available through this Service.

2. Purpose of the Service

Our service includes a comprehensive consultation to identify gaps and opportunities, resulting in a detailed project plan with timelines and cost analysis. Our individualized plans comprise quality services to help you achieve your goals efficiently.

3. Sources of Personal Data

We collect personal data from the following sources:

  • Directly from you: Information you voluntarily provide while entering your personal details on the Service.

We do not collect personal data about you from third parties or agents acting on your behalf.

4. Information We Collect and Why

We only collect the necessary information to undertake specific information processing activities, which include:

  • Non-personal information: Such as pages you have accessed, which helps us understand user behavior and improve our service.

Registration Information:

  • Email address: Used to send a welcome email, service-related communications like password resets, and email verification. We do not send marketing or third-party messages unless explicitly consented.
  • Name/Username: Allows posts and comments to be attributed to you. It will be visible on your public profile page. You may use a real or fictitious name.
  • Photo: Optional, but if provided, it will appear next to your posts and comments and on your public profile page.
  • Password: Stored securely in an encrypted system. Password reset instructions are sent via email if needed.

Upon registration and email verification, you may post and comment in the App. Note that posts and comments are public and viewable by non-registered users on the App and public web pages.

You can also send friend requests to other users. Accepted requests allow for private one-to-one and group messages, which are not monitored.

As you use the App, we track sections you visit to highlight relevant content and understand App usage patterns, helping us improve the service. This data is aggregated and not shared externally.

We record the last IP address you used to access the service for security purposes. This includes checking the approximate location (country and city) of the IP address.

Providing your personal information is voluntary. If you do not provide the necessary data or withdraw your consent, you may not be able to access or use the App.

We retain your personal information as long as you are a registered user and comply with legislative and regulatory information retention requirements. Your personal information will be securely and permanently deleted when no longer justified or upon your request.

Sharing Personal Information: Personal information collected may be shared with the Artist, the Artist’s record label, or service providers contracted for site operations or promotional activities. These contractors are obligated to use the personal information solely for these purposes.

This Service and its offerings are not directed at users under 13. If a user under 13 provides personal information without parental or guardian consent, please contact us immediately (see Section 11) for appropriate action.
